Since you are in the first year of the Health Science Pre-Professional Program at SICTC, you should pay close attention to any dual credit coursework that you have registered for through Ivy Tech. USI's HP 211 is equivalent to Ivy Tech's HLHS 100 and would be a duplicate course for you if you already have earned this credit through Ivy Tech or are registered in it. If you have questions, connect with your teacher for additional information.

Sophomores: To enroll in CAP, you must have a cumulative GPA of 3.0, unrounded, on a 4.0 scale and permission from a counselor or instructor (collected by CAP Office on your behalf). Some CAP courses have additional course prerequisites.

This information is confidential. Therefore, if you wish to allow your CAP instructor to discuss your CAP grade or CAP academic progress with your parents or other designee, or if you wish to allow USI employees to discuss your billing with your parents or other designee, you must agree to the statement below and specify who is authorized on your behalf.

A summary of your course request will be sent to the email address you provided. This is not proof of enrollment, but instead is a record of your request to be registered in the CAP course.
